A dedicated physiatrist per facility: Our providers cover one facility per day 

• Increased time and visibility in the facility allows our physiatrists to build relationships with the patients as well as their families increasing patient satisfaction.

• In partnership with the medical director and administration, our team can gradually develop rehabilitation interventions to optimize patient's outcomes including but not limited to prevention of: deep venous thrombosis, pressure injuries, contractures, falls.

• Development of an individualized rehabilitation program which may include medication management, assistive devices, orthotics, and minimally invasive procedures such as trigger points, joint injections or chemo-denervation.

• Proficient managing a diverse rehabilitation population including orthopedics, neuro-rehabilitation and pain conditions.

• Our physiatrists will attend weekly progress, insurance and Medicare meetings. 

• Optimization of transition-of-care back to the community with the highest level of function and independence possible.

• Our services are provided with no added expense to the subacute/long term facility.
